 Welcome to Amazing Smiles Family Dentistry! 
We are dedicated to providing our community with high-quality dental care in a friendly, comfortable environment. To help you get the care you need, we will make your new patient experience as pleasant as possible. We want you to tell us about any concerns you have about your care or dental health, and ask any questions you might have about your treatment program or procedures. The more you communicate with us, the more we can help you.

  • Your First Visit

    In your first visit with us, Dr. Chubb will complete a thorough examination of your dental and oral health, including detection of cavities or gum disease, a bite alignment check, and oral cancer screening. 

    Many patients will receive complete teeth cleaning during that visit. If a patient needs more extensive cleaning, a separate appointment will be made for that service. You can expect this first visit to last about an hour and a half. 

  • Patient Registration

    You may preregister with our office by filling out our secure online Patient Registration Form. 


    After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information.


    The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

  • Anxiety and Fear of Dentist

    We realize that many people experience anxiety or fear when they visit the dentist. We want to make your visits to the dentist as positive and stress-free as possible. 

    With some patients, staying attuned to their comfort and explaining the next step of each treatment is all that is needed to make their procedures comfortable. 


    When more assistance is needed, we can offer sedation with nitrous oxide. This is a sedative that acts quickly and helps calm a person during treatment. They will remain conscious and able to speak throughout the visit and the effects will wear off quickly as soon as treatment is done. Please tell us about your concerns so we can discuss your options.

  • Payment and Insurance

    After your examination, we will inform you of any further treatment needed to preserve your teeth or gums and help you understand your options. The financial coordinator will discuss payment options and assist in scheduling your appointments.


    We accept insurance from most carriers, however dental benefits vary greatly. Our staff will assist you in maximizing the value of your insurance. As a courtesy, we will file your insurance claim for you, however, you are responsible for deductibles and payments not covered by your carrier.


    Please understand that the dental professionals at Amazing Smiles recommend treatment based on your health needs, not your insurance coverage. Insurance policies generally cover only a portion of the total treatment cost. Here is a list of some of the insurance carriers we accept:

    We also offer our patients our own in-office coverage plan for those that do not have insurance. To view the details of this plan, click here. 


    As an extension of the Amazing Smiles Dental Plan, we also offer a special plan for children 12 and under. This plan is available to parents/guardians who have decided to take advantage of our Amazing Smiles Dental Plan and wish to extend those benefits to their children.
